APPLICATION FOR PERMIT WORK
Please follow these instructions:
- Download application form HERE (2 pages)
- Print & fill out the form
- Mail (only) the completed form to the Local 118 office:

You must fill out the Emergency Contact Form prior to starting work.
Please keep your contact info up to date. Send any change of address, phone number, etc
to admin (at) iatse118.com (Note: Replace (at) with @).
If you have been dispatched for work for the first time, you must fill out
TD1 tax forms (both Federal and BC) prior to starting - please include your phone number
in the top right corner of the TD1 forms. You can either scan these completed forms
and email them to admin (at) iatse118.com or bring them with you to the job site and
give them to your supervisor. Please also fill out the Emergency Contact form.
Cheques will be available in the Union Office every Friday from noon to 5.00PM. Members who are unable to come to the
Union Office on Fridays should contact the office to make alternate arrangements.
Avoid the hassle of picking up your paycheck. Local 118 offers direct deposit to non-members, as well as members.
To setup Direct Deposit, please complete the following IATSE 118 form Direct Deposit form and provide either a VOID CHEQUE or ask your bank to print a direct deposit form.
Email the completed direct deposit form AND either a scan of your VOID Cheque or form from your bank to admin (at) iatse118.com (Note: Replace (at) with @). You will be notified once direct deposit has been setup.
